H&S 1 - Introduction to the Incident Form Builder

The Incident Form Builder gives you greater flexibility when creating and maintaining your incident forms. Whether you're setting up a new incident type, updating existing questions, or tailoring forms to your organisation's processes, the builder makes it easy to manage everything in one place.

Designed with the same intuitive experience as Checklists, the builder allows you to organize questions, create logical workflows, and ensure staff capture the information that matters most.

This guide will walk you through the key features of the builder, how to create and manage incident forms, and what to expect when transitioning from existing forms.


How To Get There

Health & SafetyIncident Type

From the Incident Type tab, you can view all existing incident forms. To create a new one, click the green "+ New Incident/Accident Type" button.


What's new in the Form Builder

👉 Drag-and-drop questions

Build and reorder questions the same way you do in Checklists. No technical setup needed.

👉Conditional rules

Questions only appear when relevant. Keep forms focused and easy for educators to complete.

👉 Colour-coded groups

Organise sections clearly so everyone knows exactly where each question belongs.

Blue sections — incident details shared with parents

Pink sections — internal investigation notes

Colour coding makes it easy to see at a glance which section you're adding to. This keeps parent-facing information and internal records clearly separated, so your documentation is always audit-ready.


Your existing forms still work

👉 Version 1 (V1)

Your existing forms

All existing incident forms continue to work exactly as before. No action required.

👉 Version 2 (V2)

The new builder

New forms uses the same builder and UI as the checklist. You can also convert existing V1 forms when you're ready.

V1 and V2 forms run side by side. Transition at your own pace. Nothing breaks.


App Navigation

👉 Navigation at the bottom of your screen

In the 1Place app, the navigation bar now sits at the bottom for Health & Safety, Checklists, Tickets, and Hazards. This gives you more screen space to focus on what matters. The experience is now consistent throughout 1Place.
