To return a DVD order:

Contact Evergreen Wellness by submitting this form to receive a Return Authorization Number (RA#). Click here for the form »

An illustration of this step in the form is shown below:

  • Fill out your name, email address and select the radio button for “Return Authorization”
  • Provide a summary of why you were unhappy with the program (in the Comments/Questions box) so we can learn and improve.
  • We will email you back with your RA#.
  • Write your RA# on your original packing slip or on a copy of your invoice and put it in a return envelope along with your DVD.
  • Your return package must be postmarked within 15 days of your original purchase date.

  • Keep a carrier receipt and tracking number in case the package gets lost in transit.
  • Once we receive your return package, you will receive your refund within 5 business days. 

PLEASE NOTE: we are not responsible for packages refused for delivery, returned to sender, insufficient postage, sent to an invalid destination or sent without an RA#.
