Safety Reports are a useful way of generating data from incidents and near misses recorded over a specified time frame. They distinguish the top five contributory factors and categories related to both incidents and near misses over the period investigated. Hence, generating a Patient Safety Report is an excellent way to identify key learning points and prioritising ways of improvement.
The steps outlined below demonstrate how to access and download a Patient Safety Report.
Select 'Safety Report'. This can be found across the top menu bar on the Medicaiton Safety Event dashboard.
2. Select the period of time for which you would like to generate the report from, defaulting to the last month.
3. Select 'Find incidents'. This will bring up a list of all incidents that are relevant for this period. Select the incidents you would like to be included in the report by ticking the boxes on the left-hand side. Then click 'Generate Word Document'.
4. Now, give your report a name and fill in all mandatory fields. Drop down menus are available with possible answer suggestions.
Note: if your answer is not provided as an answer, just press 'Other'. An empty space bar will show which will allow you to type in your response.
5. Finally, click 'Download' and your report will be available to view as an excel file.
