Incident Report Form Explained
Please Describe What happened is a short summary of what happened including details such as the date and time, situation, and harm level incurred by the patient/resident.
Patient/Resident Details allows you to record patient specifics.
What happened/Event Category Incident Type these categories are broken into "Medications and Directions" and "Clinical and Other".
Incident Details Split into "Situations with Risks", and "Outcomes with Risks". Also who identified the error, and the date and time it occurred.
Risk and Harm Details Auto-calculated risk score based on questions relating to harm level and potential recurrence.
Details of medication/ devices involved section involves the medication(s) which were involved the incident.
Stages Stages Involved/When the incident happened, primary stages include "Prescribing", "Non-Automated Dispensing/Preparation", and "Supply/Ordering/Storage".
Once you have recorded all of the incident details, click the green 'SAVE' button at the bottom of the form.
You can also save the event as a draft if all mandatory fields have not been completed by selecting the "SAVE AS DRAFT" button.
