The "Root Cause Analysis" is a useful tool that allows you to examine the true problem which led to this incident occurring.
Pharmapod uses the '5 Why's' which is a simple, well-recognized tool for establishing the Cause and Effect of incidents, especially where human error is involved. Five iterations of asking 'why?' is deemed the optimum number to get to a root cause, but you may establish a Root Cause in less steps.ย
You can access the "Root Cause Analysis" by clicking on the event summary page of a particular incident and clicking the green "Add" button in the "Root Causes" box.
Before answering the questions, you are shown a summary of the event i.e. the Description, Countributing factors, and Risk score to familiraise yourself with the nature of the event. You also answer the following questions to see if there was a deviation in procedure whilst this event occurred.
You then answer the question "What is the problem that led to this incident": followed by filling in the five "Why" box's and inputting your root cause. After doing this, you press the green "Ok" button to save the root cause.ย
