The "Root Cause Analysis" is a useful tool that allows you to examine the true problem which led to this incident occurring.
Pharmapod uses the '5 Why's' which is a simple, well-recognized tool for establishing the Cause and Effect of incidents, especially where human error is involved. Five iterations of asking 'why?' is deemed the optimum number to get to a root cause, but you may establish a Root Cause in less steps.ย 

You can access the "Root Cause Analysis" by clicking on the event summary page of a particular incident and clicking the green "Add" button in the "Root Causes" box. You then answer the question "What is the problem that led to this incident": followed by filling in the five "Why" box's and inputting your root cause. After doing this, you press the green "Ok" button to save the root cause.ย
