Steps for completing the PSSA tool (Canada)

Go to the 'Assessments' module on the left-hand side of the main menu bar

  1. Select the blue "Start New Assessment" Button.

  2. The PSSA consists of six domains, with six to seven statements within each domain

  3. Each statement should be reviewed and if relevant, discussed with the pharmacy team 

  4. The team will be asked to rate the degree of opportunity for improvement regarding each statement using the scale below. The level of alignment to the statement that you think best represents your pharmacy’s practice should be selected (or the closest level if none fit exactly).

  5. For each statement, there is the opportunity to add notes if you wish (e.g. to record how you are achieving best practice)

  6. Please remember to click the 'Save' button before moving to 'Next Domain'

5 Point Likert Scale

Often/Always Practiced: Best Practise is Being Achieved
Indicates minimal uncontrolled risk, with excellent risk management strategies in place that are evaluated and monitored to achieve desired outcomes or goals for medication safety.  

Sometimes Practiced: Partial Opportunity for Improvement
Indicates good risk management and control systems in place, but an opportunity exists for improvement of controls or increased adherence to controls. It may also be that controls are being developed but are not fully inplace, evaluated or monitored to reduce risk.   

Not/Rarely Practiced: Significant Opportunity for Improvement
Indicates effective risk controls have not yet been developed, and a significant lack of risk control exists. Actions to improve risk management are a matter of priority to achieve desired outcomes or goals for medication safety.   

Regarding the Action Plan section, prior added actions can be completed by clicking on the Open within the "In Progress Dashboard", and SMART Action Plans section. Within the Action Plan section click the blue Mark as Complete button. Each action needs to be marked as completed in order to complete your assessment.


When all domains reach 100 % Completion rate and all Completed Actions are showing as Completed e.g. 0/0, 1/1, 2/2 etc. you need to select the blue "Mark as Complete" button to mark your assessment as Completed.

Once completed your assessment should then be visible in the Assessment History Section. Your submission then moves from the "In Progress" tab to the "Completed Dashboard".
