Pharmacy Safety Self-Assessment (PSSA) Domains (Canada)

The PSSA consists of six domains, explained below. These are constructed with a focus on CQI and implementation of concrete and tangible medication safety strategies.

  1. Comprehensive Patient Information: This domain will assess areas of collecting, updating and amending patient information. Completion of this domain and creation of an action plan will serve to optimize patient safety by ensuring that robust processes are in place in relation to accurate patient information

  2. Communication: This domain will assess pharmacy team member communication within their team, with patients and with prescribers. Completion of this domain and creation of an action plan will ensure methods of communication are in place to minimize the risk of error.

  3. Medication Storage, Preparation and Equipment: This domain will assess the storage, disposal and recall of medications and medical devices to support safe medication supply and efficient workflow. Completion of this domain and creation of an action plan will allow the pharmacy team to review and optimize the appropriateness of medication storage within their pharmacy.

  4. Training and Education: This domain will review the training of staff involved in medication preparation and supply. Completion of this domain and creation of an action plan will allow procedures to be reviewed and/or put in place that provide the pharmacy team with ongoing education and training that support medication safety   

  5. Medication Safety Event Management: This domain will assess how pharmacy team members address, record and review medication incidents and near misses, including use of tools such as the AIMS Pharmapod platform. Completion of this domain and creation of an action plan will support adherence with the AIMS program and standardize the approach to medication safety across all pharmacies

  6.  Pharmacy Processes and Continuing Quality Improvement: This domain will review certain pharmacy processes that have an impact on medication safety and service quality. Completion of this domain and creation of an action plan will ensure that processes relating to medication safety are reviewed and if necessary, improved
