Monthly PQS Report (England)

In terms of qualifying for the pharmacy quality scheme and annual PQS (Pharmacy Quality Scheme) safety report needs to be completed annually.
You can complete this by following these steps:

1. Click on the "Incidents" dashboard followed by "PQS Safety Report" which is located between the "Dashboard" and "Search" sections at the top of the page.

2. Select the "Pharmacy/Pharmcies" which are applicable and then set the date range to "Last Month". Only events which were recorded within the last month will appear.

3. A list of events which have been recorded within the last year will now appear. You then select the blue "Generate Word Document" button which is located on the right-hand side of the page.

4. The 6 sections which need to be completed in order to complete the report appear. You need to input all of the numbers in the table for section 1 on a per monthly basis. Numbers will auto populate for the "Number of Incidents/Near Misses Recorded". 

You can obtain numbers for the "Number of Near Misses/Dispensing Incidents involving high-risk LASA" and the "Number of Prescribing Incidents" within "Analytics" "Export Data" and setting the appropriate filters. Once inputting all of this information you then select the "Report Range" as "Annual" to signify that this is an annual PQS report and not a monthly one. 

5. Complete sections 2 - 6 answering the questions and selecting "Next" each time. Once inputting the details in Section 6 you then select the "Finish" button and all of the data saves in microsoft word format.
