The new incident and near miss forms launched as of 2023 have updated CQI tools which are surmised below:
Contributing Factors:
Moved from the forms itself to the event summary page as a separate widget.

Root Cause:
New format, questions from the form populate into this section i.e. Stages Involved, What happened, Describe What happened and the Contributing factors widget answers populate. As a result of the relevant information feeding in, the user can complete the 5 Why's tool and generate an appropriate root cause.

Scores auto-calculated based on answers given within the event forms to the following questions:

A SMART action plan must be added to the action. A template is accessible prior to adding the action. You can also set the action to be "must be completed before the event is closed". Previously, the action had to be completed before an event could be closed and there was no option to prevent this system behaviour.
