How to report an LFPSE Medication Incident (England)

Once logged into Pharmapod, you can report an LFPSE Medication Incident by clicking on the form icon within the home dashboard.

From here, you are directed to the form.

All mandatory fields have a red asterisk beside them e.g. "Describe what happened"

Field within the NHS logo beside them are the ones which send to the NHS via the LFPSE integration.

Skip logic is included in the form e.g. in the below screenshot for the "What happened" question, when selecting "Administration time (HOA)/timing incorrect", additional options show with an additional question as follows: "Administration time (HOA)/timing incorrect"

A "Risk and Harm Details" section has been added to the form. After answering each of these questions, the Risk Matrix score is auto-caculated. This score can be viewed within the event summary page for the event (on the right-hand side), within the Risk Matrix section.

Stages When it Happened section, primary options include: Prescribing, Administration, Automated Dispensing/Preparation. Sub-categories clickable aswell.

Learning Points and Further Details Section is now part of the form and not just within the event summary page (CQI Tools on the right-hand side).

Select save once the event is completed, or save as draft if not all mandatory events have been completed and you need to come back and finish it.
