Enhancements

Safety Reports (Not PQS)

1. User Story 11287: GRLS > Safety Reports > Getting 524 Error, When large dataset is pulled.

• Description: Within the Safety Reports section (not PQS Safety Reports), when searching for events and there is a large subset of data e.g. more than 10 events, the view will be sorted in a table format by rows of 10, similarly to Cases.

Form Settings, Closure Rules

2. User Story 11193-11195: Form Settings - Floating Forms > Add new configuration - Closure Rules

• Description: Within Form Settings, clicking on the relevant form, and in the “Add/Manage Configurations” section, a new field is added called “Manage Closure Rules”. From here, the user can add specific closure rules and assign certain users either manually or by facility role e.g. Store Supervisor, Designated Manager etc. to be able to close cases. Once setup, users not included in the rule cannot close cases. His access level is only available to users with Form Setting, and Org Level access assigned. Similarly to the “Notifications” section, previous closure rules setup for each form will show under the “Manage Closure Rules” section.

3. Task 11388: What Happened Section - Category "Change in medical condition eg. severe unresponsive hypoglycemia"

• Description: Within the incident and near miss forms for the What Happened Question, and the “Change in Medication Condition” option, a new sub-option has been added "Change in Behaviour-risk to self or others".

4. User Story 11276: Medication Incident & Good Catch Updates - What Happened Section, Privacy Breach Question

• Description: Within all forms the following changes have been made with respect to the “What Happened” question and “Privacy Breach” option:

1. Keep the 4 selection options for "Privacy Breach" under the question "What happened (select all that apply)"

2. Update "Privacy Breach" to "Privacy Breach (eg. personal health information disclosed)

3. Update the sub-category question under "Patient/resident incorrect" to include:

a. Medication received by incorrect patient/resident (e.g. given to the wrong person/patient crossover, delivered to the wrong address) (keep the same)

b. Sent or received incorrect patient/resident document/report/memo/drug information (keep the same)

c. "Patient/resident name incorrect in system rephrase only eg to (eg. name not added correctly, incorrect profile selected)



The above requirements are highlighted in Bold italics, are the updates which we need to do in the What Happened Section. The updated What Happened mock-up is added to the attachments.



Under the Incident Details Section for the form, we need to make the below changes.



1. If option a) and/or b) is selected above, then auto populate "resulted in privacy breach" under "Incident Details" AND direct the user to choose one of the 4 selection options for "Privacy Breach" under "What Happened (event category/incident type) (e.g. disclosed PHI without authority, lost PHI, stolen PHI, used PHI without authority) as a new branching question for selection

2. If option c) is selected, then "resulted in privacy breach" does not auto-select ok and no "privacy breach" box is populated

3. Rename "How many were impacted by this privacy breach?" to "How many individual's PHI was divulged in the incident?" AND add a drop-down selection of numeric values instead of free-form answers: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, or other (free-form box shows up)
