Why is it required?
This questionnaire collects the necessary information to process and approve market access appointments. Since market access-appointed agencies will be binding business under Bold Penguin's master code with the Carrier, we require agents and their agencies to be in good standing.
Compliance Questionnaire details
Agents must provide the following:
Current carrier appointments
Annual small business premium
Lines of business you currently write (commercial lines)
Small business retention percentage.
Top 3 commercial carriers and accompanying loss ratios for the last 3 years.
Principal agent NPN
Agency NPN*
Copy of your error & omissions dec page or COI
ACH and W-9 information
*If your agency is not licensed and therefore does not have an NPN, you will be approved for Market Access appointments as a sole proprietor. Therefore, you must provide a W-9 with your name and SSN, and your E&O verification must list you as a named insured.
Compliance Questionnaire review:
Compliance Questionnaires are reviewed daily by Bold Penguin's Compliance team. Approval typically takes between 1 and 3 business days; however, this timeline may be extended if compliance information is incorrect or missing.
