How to Complete a Survey (for participants)

How to Complete a Survey as a Participant

When you’re invited to take part in a survey, you’ll receive an email on the survey launch date with a link that takes you directly to your account, where you can complete the survey.

Accessing Your Survey

You can access the survey in two ways:

  1. From the email link – click the link in your invitation email to go straight to the survey in your account.

  2. From your account directly – log into your Shapes account and open the Assigned Tasks section.

Once in your Assigned Tasks:

  • You can filter by task type by clicking the filter icon on the top right and selecting Type → Survey.

  • This will show all surveys assigned to you.

  • You’ll see whether each survey is incomplete or overdue (with the number of days overdue displayed).

Completing the Survey

  1. Click on the survey to open it.

  2. You’ll see:

    • The survey name

    • The timeline and due date

    • A description (if provided)

    • The visibility setting (either Visible or Anonymous)

    Visibility explained:

    • Visible – your responses can be linked back to you.

    • Anonymous – your responses are collected confidentially and cannot be traced back to you.

  3. Click Complete Survey to begin.

  4. On the left-hand side, you’ll see the list of questions.
    You can:

    • Type open-ended responses

    • Select single or multiple options

    • Answer rating or scale questions

  5. Once you’ve finished, click Submit Survey.

After Submission

After submitting, your survey status will automatically update to Completed in your Assigned Tasks section.
