Instructions for Accessing, Saving, or Printing the Payment Authorization Form
1. Open the Payment Authorization Form
Access the Payment Authorization Form on Google Docs here: https://docs.google.com/document/d/121MmE62vdb2TKIBMuu-4jO-azOnuo1GjVhdZw_MF_2k/edit?usp=sharing
2. Select one of the following three methods
Follow one of the three methods below based on your preference.
2a. Save a Copy (Optional)
To make edits or save the form to your personal Google Drive, follow these steps:
Click on File in the top-left corner of the screen.
Select Make a copy.
Choose a folder in your Google Drive and click OK.
You can now edit and save your changes directly in Google Docs.
2b. Print the Form
If you prefer to fill out the form manually:
Click on File in the top-left corner of the screen.
Select Print or press Ctrl+P (Windows) or Cmd+P (Mac).
Choose your desired printer and settings, then click Print.
2c. Save as a PDF (Optional)
If you need to fill the form electronically or email it later:
Click on File in the top-left corner of the screen.
Select Download > PDF Document (.pdf).
Save the PDF to your computer.
3. Complete the Form
Fill out the form, either electronically in the downloaded copy or by hand after printing.
4. Email the Completed Form
Once the form is completed, email it to accounting@acenda.com.
Ensure the subject line includes “Payment Authorization Form” for quick identification.
