Instructions for Accessing, Saving, or Printing the Payment Authorization Form

1. Open the Payment Authorization Form

2. Select one of the following three methods

  • Follow one of the three methods below based on your preference.

2a. Save a Copy (Optional)

  • To make edits or save the form to your personal Google Drive, follow these steps:

  • Click on File in the top-left corner of the screen.

  • Select Make a copy.

  • Choose a folder in your Google Drive and click OK.

  • You can now edit and save your changes directly in Google Docs.

2b. Print the Form

  • If you prefer to fill out the form manually:

  • Click on File in the top-left corner of the screen.

  • Select Print or press Ctrl+P (Windows) or Cmd+P (Mac).

  • Choose your desired printer and settings, then click Print.

2c. Save as a PDF (Optional)

  • If you need to fill the form electronically or email it later:

  • Click on File in the top-left corner of the screen.

  • Select Download > PDF Document (.pdf).

  • Save the PDF to your computer.

3. Complete the Form

  • Fill out the form, either electronically in the downloaded copy or by hand after printing.

4. Email the Completed Form

  • Once the form is completed, email it to accounting@acenda.com.

  • Ensure the subject line includes “Payment Authorization Form” for quick identification.
