With Form Fields in Appstle Memberships, you can easily gather preferences, instructions, and details directly from customers at the time of purchase.

Form Fields help you:

🧩 How to Add Form Fields to a Membership Plan

  1. Go to Appstle Memberships → Edit Membership Plans.

  2. Scroll down to the Additional Questions section.

  3. Click ➕ Additional Questions.

  4. Select the desired form field type from the dropdown.


Types of Form Fields You Can Add

📝 1. Text Field

Use this field to collect short, free-form input from customers.

Best for:

  • Delivery notes

  • Gift messages

  • Special instructions

Configuration Options:

  • Field Label: Text shown to customers
    Example: “Enter delivery instructions”

📋 2. Dropdown

Allows customers to select one option from a predefined list.

Best for:

  • Age range

  • Membership preferences

  • Plan variations

Configuration Options:

  • Field Label: Customer-visible label

  • Options: Enter values separated by commas

☑️ 3. Checkbox

Used for yes/no or toggle-based selections.

Best for:

  • Gift purchases

  • Opt-in confirmations

  • Field Label: Instruction text
    Example: “This membership is a gift”

Allows customers to select only one option from multiple choices.

Best for:

  • Single-choice preferences

Allow customers to select a specific date based on your business requirements.

  • Field Label: Customer-visible prompt

Understanding "Is Required" and "Is Visible" in Custom Fields

Is Required?

  • What It Does: If checked, the customer must fill out the field. If unchecked, it’s optional.

  • Why It’s Helpful: Ensure you gather essential information.

Is Visible?

  • What It Does: If checked, the field on the form is visible to the customer.


🧾 Where Form Field Data Is Stored

Once a membership is purchased, form field data is saved as Attributes.

🛍️ In Shopify

Below is the checkout page of a sample Shopify order with a form field set (text field) and filled out prior to purchase.


Go to Orders, click an Order ID, and view the custom fields under Attributes (e.g., delivery instructions).


⚙️ In Appstle Memberships

Go to Memberships in the Merchant Portal, click the Membership ID, open the ⋮ (three-dot) menu, and select Attributes


📤 Exporting Form Field Data

You can export all membership form field data as a CSV file.

Go to Memberships in the Merchant Portal, click Export, and open the file in Excel or Google Sheets. Each form field appears as its own column.
