All of these tools can be found under the Media & More tab on the right-hand side of the Form Builder Toolbox.
Accessing the Media & More Tab
Open your form in the Form Builder
Navigate to the Toolbox on the right-hand side
Click on the second tab labeled Media & More
Embed an Image
You can add static images to your form for branding, instruction, or visual context.
To embed an image:
Drag the Embed Image element into your form
Click Upload Image
Choose an image from your device and click Open

Draw Over Image
Allow users to annotate or draw directly on a static image using our built-in editor.
To use Draw Over Image:
Drag Draw Over Image into your form
Upload an image that users can interact with
Users can either draw (pencil icon) or type (letter "a" icon)
Change cursor color using the circle in the bottom-left
Tip: Add instructions with a Free Text field from the Layout tab so users know how to interact with the image.
Take a Picture
Let users capture or upload photos directly within the form.
To add this field:
Drag Take a Picture into your form
Users will see a camera icon — clicking it opens their device's camera or file upload dialog
Users can upload multiple images if needed
Draw Over Picture
This feature combines Take a Picture with drawing tools, allowing users to annotate their own photos.
To use Draw Over Picture:
Add the Draw Over Picture element from Media & More
Users upload or take a photo
Click the photo thumbnail to open the editing tools
Use the pencil or text tool to annotate
Great for inspections, fieldwork, or feedback that requires visual markup.
Take Video
Capture real-time video or upload existing files.
To use:
Drag Take Video into your form
Users click the video camera icon to either record or upload a video
Multiple videos can be uploaded if needed
Take Audio
Let users record or upload audio clips — perfect for interviews, notes, or verbal responses.
To use:
Drag Take Audio into your form
Users click the microphone icon to start recording or upload an audio file
Attach Document
Collect any file types from users — PDFs, Word docs, spreadsheets, and more.
To use:
Drag Attach Document into your form
Users click the paperclip icon to upload their files
Add a Subform

Add a Barcode
Let users upload a barcode by taking a photo or selecting one from their device.
To use:
Drag Add a Barcode into your form
Users click the barcode icon to upload or scan
A green button confirms the upload is complete
Add a QR Code
Similar to barcodes, but made for QR scanning.
To use:
Drag QR Code into your form
Users click the QR icon to scan or upload a QR code image
Add OCR (Optical Character Recognition)
OCR allows users to scan and extract text from images — especially useful for digitizing printed documents or forms.
To use:
Drag the OCR element into your form
Users tap the OCR icon to open their camera
After scanning, they tap Save
A green tick confirms it’s complete
The extracted text appears in the form submission
Note: OCR works best on the mobile app.
Add GPS
Capture the exact location of the respondent.
To use:
Drag GPS into your form
Users click the GPS icon to load their location via map
Coordinates are saved with the form submission
Ideal for field teams, inspections, and on-location check-ins.
