Here's how to upload your previous medical records to Tia:

First, reach out to your previous healthcare provider to request that they send your medical records to Tia. You may need to complete their specific release form.

If you have a copy of your records or if they're accessible via a patient portal, you can download and attach them directly in a chat with Tia by clicking on the paperclip icon. If the file is too large to upload via chat, you can email them to Tia's Care Team at care@asktia.com.

If you need assistance in requesting your previous records, message your Tia Care Team and we’ll send over a Medical Record Request form. Make sure to fill out a separate form for each office or provider from whom you wish to request records, including their clinic phone number and fax. (You may be asked to include Tia's fax number, which is (646) 871-6820.) Send the completed form back to us, and we’ll fax it to the facility or provider.
