At Augmentive, we offer a Shared Care approach designed to ensure continuous, high-quality care. Once you’re stable on your medication, your GP can take over your treatment plan and continue supporting your mental health journey. Our specialists will provide the initial titration phase and work with your GP for a smooth transition to ongoing care.
How Shared Care Works
1. Specialist Assessment and Titration Phase
Your journey begins with a personalised assessment and titration process led by our psychiatric specialists. We’ll work together to find the right medication and dosage to ensure optimal symptom management. Once stability is achieved, we’ll prepare for the transition to your GP.
2. Handover to Your GP
Once you’re stable on your medication, we’ll collaborate with your GP to transfer your care. We’ll provide them with all the necessary information, including your treatment plan and prescription details, to ensure seamless continuity of care.
3. Ongoing GP Support
After the handover, your GP will oversee your ongoing care. If your needs change or if further support is needed, your GP can consult with our specialists. This integrated approach offers accessible, local support throughout your mental health journey.
Preparing for Your Journey
Consult Your GP First:
Before starting with us, speak to your GP to confirm they’re willing to take over your care after the titration phase with our specialist.
Coordinated Care for Peace of Mind:
Once you’ve completed the titration process, we’ll provide your GP with a full handover report. This will enable them to manage your ongoing care in line with your personalised treatment plan.
Is Shared Care Always Available?
While most GPs in the UK do accept Shared Care agreements, we are aware that some GPs have a policy of refusing them, often due to NHS wait times. Unfortunately, we are unable to influence their decision. It’s essential to confirm with your GP before starting the process to ensure everything runs smoothly.
