Reactivating a Discontinued Medication

Reactivate a discontinued med order.

Managing discontinued medications effectively is crucial for ensuring accurate records and proper medication administration. This guide provides steps for reactivating discontinued medications.

Step 1:

Navigate to the "Medications" tab in the resident's chart.

Step 2:

Scroll to the bottom of the active medications list to find the Discontinued section. Click "Discontinued Orders" to see all discontinued medications.

Note: The "discontinued orders" dropdown button will only appear if the resident has discontinued orders. If there is no dropdown, it means there are no discontinued orders for that resident.

Additionally, certain conditions may prevent reactivation, such as medications scheduled for only one day or those automatically discontinued due to a DC message from the pharmacy.

Step 3:

Select the medication you wish to reactivate. If you have several orders with the same name, please pay attention to the end date on the discontinued order, as it indicates when it was discontinued. Use this information to select the correct medication order.

Step 4:

Once you have clicked into the appropriate medication order, click the "reactivate" button on the bottom left of the screen.

Step 5:
Click "Confirm Reactivation."


For eMAR users- After confirmation, a pop-up will appear outlining the next 24 hours of med passes that have been added to eMAR.

Step 6:

The reactivated medication will appear in the active medication list in the resident's chart with a "New" tag in the title. The original start date will stay in the med order, but the end date will be empty.

Notes

If a medication has an active order instead of a "reactivate" button you will see a "View Current Order." Instead of reactivating you will be taken to the active order to review.
