How to document vitals in Care Track

You can document vitals either as a full panel or individually inside Care Track. Here’s how to add them from the Care tab in the resident’s chart and how to document them inside Care Track.

If you do not see Vitals in the Care tab, it may not be enabled for your community. If you would like more information, please reach out to August Health Support for assistance.

Adding Vital Orders to Resident's Routines

Step 1:

Navigate to the "Care" tab inside the resident's chart.

Step 2:

Click "+ Add Routine" and select "Vitals."

Step 3:

Enter the required information for the vital routine. Select the needed vitals from the dropdown, click “+ Add a Vital” to add more, and use the icon on the right to remove any.

Step 3:

Click "Save," and the vital order will be added to Care Track.


Documenting Vital Orders in Care Track

Step 1:
Navigate to the resident in Care Track, click "Chart" on the vital routine.

Step 2:

Enter the vitals and click “Submit.”

If none were taken, select “No vitals to chart.”
If only some were taken, submit what you have and return later to complete the rest.

When vitals orders are complete, a "Complete" tag shows on the routine.

If not all vitals are completed, the routine will show how many have been taken out of the total scheduled.

If you select “Yes,” you can mark exceptions. Any vitals not taken that day should be marked as exceptions.

When vitals are recorded in Care Track, they can be found in Care Progress and the vitals sections of the Notes tab in the resident's chart.
