This article outlines how to use our Intelligent Service Plan. It covers how to get started, schedule interventions, and other key information you need to successfully create and manage an Intelligent Service Plan. By instantly transforming resident assessments into clinically rigorous, state-compliant frameworks, Intelligent Service Plans establish a built-in quality baseline that ensures community standards and regulatory requirements are met from the start. This automated efficiency cuts plan creation down to minutes, empowering both new and experienced caregivers to shift their focus from tedious documentation to personalizing the specific care details that truly matter to each resident.

Step 1:
After completing the Resident Assessment, go to the Tasks tab and choose Service Plan. After you complete an assessment, you'll be prompted to open a new service plan if one is needed. If the service plan is due based on a schedule, it will automatically open. If needed, you can also create a new service plan from the New Documents list in the Documents tab.
Step 2:
Starting in the Key Care Themes section, you can review the interventions generated from your most recent assessment responses. As you go through each section, you will select the performer and frequency for every item. You can also click the "+" icon to add notes and include any additional details specific to that Key Care Theme or ADL.
Note: Only items assigned to a caregiver will populate in Care Track. If multiple roles are selected, the intervention will still appear in Care Track as long as a caregiver is included.
What are Key Care themes?
Key Care Themes help organize a resident's service plan by grouping related goals and interventions under a broader care need or risk area, such as Fall Risk. Rather than focusing on a single ADL, Key Care Themes can span multiple areas of care and provide a more comprehensive view of a resident's needs.
Each Key Care Theme contains one or more goals, along with the interventions used to support those goals. This allows care teams to create actionable care plans with clear desired outcomes while keeping related care needs organized in one place.
Step 3:
Review your ADL's. Start by confirming the person responsible and frequency for the intervention.
During this process, you can delete interventions as needed. If an intervention is linked to a key care theme, it must be deleted within the Key Care Theme section. If you don't see a trashcan icon in the ADL section, this is why. If the intervention is standalone (not linked to a key care theme), you can delete it directly from the ADL section.

Step 4:
In the Scheduling section, you can schedule care either in bulk or by individual routine.
Note: If an intervention is not scheduled, it will not appear in Care Track.
Step 5:
Click Review and Sign once all information has been completed.
Interventions update in Care Track as soon as the first community signature is applied. Even while awaiting a Responsible Party (RP) countersignature, your signature ensures the team is immediately providing up-to-date care.
Important Note: While you can manually add additional key care themes, goals, and interventions, doing so may indicate that the resident is due for a formal reassessment.
Updating an intelligent service plan directly without reviewing the resident’s assessed needs creates a discrepancy between the care provided and their official records. This misalignment can have significant staffing and billing implications for your community.
