Resident Assessment Key Points:

  • The August Health team works closely with your community leadership to customize your community's assessment.

  • While August Health pre-populates many of the needs typically found on a resident assessment, each section can be further customized via the “Additional details” free text box.

  • As the assessment is completed, the score and level (found at the top of each page) dynamically change to reflect the inputs.

  • At the end of the assessment, there is an opportunity to review the answers before finalizing.

    • August Health will also automatically highlight any incomplete sections.

  • The completed version of the Assessment lives in the Documents tab.

  • When you update the assessment in the future, August Health will automatically repopulate your previous answers so you can easily make amendments.

Service Appraisal Key Points

  • The Service Appraisal takes all of the identified needs from the assessment and automatically populates it into the Service Plan to guide your responses.

  • August offers preset service plan options for you to choose from.

    • In the Service Plan’s section, click “Add preset plans” next to “Objective/Plan” to view the options.

    • We encourage you to personalize the service plan in the free text box under “Objective/Plan”.

  • At the end, there is an opportunity to review your answers before finalizing the Service Plan.

  • Once the document is finalized, relevant staff and family members will be notified to sign.

Where can I view the Service Plan points breakdown?

You can view the Service Plan points breakdown by opening up the finalized Service Plan document from the "Documents" tab. At the end (typically the last page or two), there is a full breakdown of the points assigned to each assessment category and the resident's care level.

Where can I view the current and historical Resident Assessment and Service Plans available?

You can view the current and historical Resident Assessment and Service Plans in the Documents center.

  • Go to the resident's profile and open the "Documents" tab

  • Select your desired document

  • On the right hand side, click the link “View archived versions”

  • Select the relevant date

  • That version of the document will automatically open in a separate tab.

What can I do if the family has not signed the Service Plan?

If the family has no signed the Service Plan and you have already sent a few reminders, you can mark the Service Plan as done. Here are the steps:

  • On the Service Plan’s Task line, there is a link called “Mark as done”.

  • Once you click this link, a popup will appear requesting a reason for why you are completing this without a signature.

  • Provide some context and save

  • The Service Plan Task will be saved and the finalized document will be moved to the Documents tab.

Once your residents have a completed assessment, you'll be able to create daily Caregiver Summaries for them using the Care tab.
