After a cognitive assessment, patients and families need more than a score. They need to understand the diagnosis, know what to watch for, and have a clear picture of what comes next. The BrainCheck Care Plan is designed to give them that, while reducing the manual work it takes your team to put it together.
If your team has used the Care Plan before, this version works a bit differently. The Care Plan is now built around an updated set of functional and behavioral assessments. Those results, along with cognitive assessment findings, are pulled into a new Combined Results Summary, a single page that highlights areas of concern in one place rather than opening a separate report per test.
The new Care Plan is only available to customers on BrainCheck v2. If you’re not sure which version your practice is on, or would like to learn more about access, reach out to us at support@braincheck.com or call (888) 690-0977.
When it's used
The Care Plan is generated after a diagnosis has been confirmed in the system by the clinician. The typical sequence looks like this:
A cognitive concern is identified, and the patient completes a cognitive assessment.
If results fall outside the expected range, a care partner is identified, and functional and behavioral assessments are completed.
A clinical workup is performed as needed, including labs, imaging, or referrals.
All assessment results are consolidated into a combined results summary. Any additional workup results are reviewed alongside this summary.
The clinician reviews the combined results summary, any additional workup, and applies their own clinical judgment to confirm or adjust the suggested level of cognitive function in the system.
Once the level of cognitive function is confirmed in the system, the Care Plan is generated using information already collected in BrainCheck.
What's included
Sections are generated based on the patient's information and the clinician's determination. Not every section appears in every plan.

How it works
Once the clinician confirms the diagnosis in the system, the Care Plan is generated from the information already collected in BrainCheck. The clinician can then:
Preview the generated plan and adjust the suggested level of cognitive function based on clinical judgment, which updates the care plan output accordingly.
Download the plan as a Word document for further edits before sharing.
Approve and share the finalized plan with the patient and care partner.

Frequently asked questions
Do we need to change how we use BrainCheck to use Care Plan?
One additional step is required. To generate a personalized Care Plan, your team will need to complete functional and behavioral assessments in BrainCheck in addition to cognitive testing. These assessments are what drive the plan's content. Because everything is collected in BrainCheck, all results are consolidated into a single summary automatically.
Is the Care Plan automatically shared with patients?
No. The Care Plan is reviewed and approved by the clinician before it is finalized or shared. Clinicians can preview, edit, and approve the plan before it goes to the patient.
Can clinicians edit the Care Plan?
Yes. Clinicians can adjust the suggested level of cognitive function, which updates the care plan output accordingly. The plan can also be downloaded as a Word document for further edits before sharing.
Does Care Plan cost anything extra?
Access to Care Plan depends on your current BrainCheck subscription. Reach out to support@braincheck.com with questions about your plan.
