Tele-Primary Care | Preventive Care

Tele-Mini-Visits

  • Allergy

  • Bronchitis

  • Cold/Cough

  • GERD/Gastritis

  • Headaches/Migraine

  • Sprains

  • Rashes

  • URI (Upper Respiratory Infections)

  • UTI (Urinary Tract Infections)

Complex chronic patient care with Tele-Primary Care & Tele-Specialist

  • Type 1 Diabetic (stable)

  • Oxygen-dependent COPD/Asthma

  • Chronic congestive heart failure

The following adult patients are NOT eligible for telemedicine services and would be best served with an onsite team:

  • Animal or human bites

  • Cancer treatment

  • Requiring immunosuppression therapy

  • Organ transplant

  • Hospice patient

  • Hospitalized patient

  • Requiring IV or IM medications

  • Requiring controlled substances and monthly monitoring

  • Pregnant patients

  • Mental health conditions such as but not limited to ADHD, OCD, severe anxiety, severe depression, bipolar disorder, suicidal ideations, and schizophrenia

  • Substance abuse

The following children patients are NOT eligible for telemedicine services and would be best served with an onsite team:

  • Under the age of 5 years

  • Mental health conditions such as anxiety, depression, ADHD, OCD, and learning disability such as dyslexia

  • STIs
