Cooperators claims can be submitted two ways.  

You can go online to www.cooperators.ca and download a claim form to submit by mail.

Call the Cooperators at 1-800-667-8164 to request a temporary password. This should work in both the web, www.cooperators.ca, as well as the mobile app, Benefits Now. 

Through a secure login, you can access general information about your coverage and specific information about your extended health care, dental and health spending account claims.  You'll also find information about dependents, printable Extended Health Care and Dental claims forms, and a glossary of insurance terms.  You can also take advantage of e-claim submission of your health and dental claims through Benefits Now.

You can also submit claims with your mobile by downloading the mobile app.
