There are two options for submitting your Orthotics claim.

Option 1:  Have someone do it for you.

Ask your practitioner and/or supplier who prescribed the Orthotics if they will submit the claim on your behalf, directly to GSC.  This way you won't have to worry about any paperwork or paying out-of-pocket for the service (except for the portion not covered by your benefit plan).

Option 2:  Do it yourself.

If you have to pay your practitioner and/or supplier directly, you will need to submit the claim yourself.  If the claim is for a  practitioner and/or supplier, as in this case being a practitioner and/or supplier, you can submit the claim online via Plan Member Online Services.

  • Visit greenshield.ca to print the claim for you need. You can even set up pre-filled personalized claim forms.

  • Complete the claim form and sign in the appropriate spot.  Remember to include your GSC ID number (found on the front of your ID card)

  • Include your original receipts and remember to take a photocopy of for your records

  • Provide your full mailing address

  • Send the form to GSC address indicated on the form.

Note:  the claim must be received by GSC within 12 months from the service date (unless stated otherwise in your benefit plan documentation).
