What I.D.'s do I need if I am a patient/caregiver?

You will need your patient's or caregiver's temporary or annual registration card and proof of identification. The identification should have a photo, date of birth, and expiration date.

It can be one of the following:

  • a driver's license

  • a government-issued identification card

  • a passport

  • a military identification card

We can accept MA temporary paper IDs as a valid form of ID as long as they are accompanied by the expired I.D. it is replacing. We do not accept digital versions of an I.D.
