Assigning Assessment Forms

To begin, navigate to the desired assessment within the Forms module.

Forms can be assigned manually or by using a QR code and shareable link. Instructions for both methods are outlined below.

Manual Form Assignment

Select the form and go to the Assigned Members tab. Click Assign Form.

In the pop-up window, select the desired users. You can filter by team and user role to make selection easier.

Once users are selected, click Save. You’ll then be prompted to set a Start Date and End Date.

  • Start Date: Determines when the form will be available to users. This is helpful for pre-scheduling assignments.

  • End Date: Specifies the last day the form is available. If not completed by this date, the form will no longer appear in the user’s platform.

Bridge Tip: Since this form may be used indefinitely, we recommend not setting an End Date. This ensures that new users can be added and complete the form in the future.

From the Assigned Members tab, you can also set due dates individually or in bulk.

Make changes in bulk by clicking Bulk Actions.

Make changes individually by clicking on the calendar icon to the right of the user's name.

On the final pop-up, choose whether to trigger an assignment notification. If selected, users will receive an email and a push notification (if they have the mobile app installed).

Click Assign to complete the process. A confirmation message will appear once the form has been successfully assigned.

QR Code & Link Form Assignment

Select the form and navigate to the Settings tab.Click Share Invite Link.

Choose the validity period for the link (from 7 days to 1 year), then click Create Link.

Copy the link and download the QR code. Based on your preference, check or uncheck the “Send me an email” option. Click Done to finish.

Note: Every time a user clicks the link or scans the QR code and opts into the form, they’ll automatically appear in the Assigned Members tab. From there, you can:

  • Track their completion status

  • Send reminders

  • Adjust the form’s end date

  • And more

To deactivate the link or QR code at any time, return to the Settings tab and click Deactivate link.

Manage Form Assignment

You can add new teams or individual members to an existing form assignment at any time. Just return to the Assigned Members tab to manage your list and make any necessary updates.

Tracking Completion

Once the form is assigned, you can monitor completion status from the Assigned Members tab. As users submit their forms, their status will automatically update to show whether the form has been completed.

Important Considerations for Athlete Users:

Athletes can access assigned forms by logging into Bridge via the web or mobile app and navigating to the Today section, where all active forms will be listed.

If using a QR code or shared link, the athlete will be prompted to enter their email address:

  • If already registered, they should use the same email linked to their Bridge account.

  • Upon submission, they will receive an email with a “View Form” link that opens the form directly in a web browser—no login required.

This method works for both registered and unregistered users.
