BridgeAthletic forms can be accessed quickly via a QR code or a web link. Whether you're a new athlete or have been using Bridge for years, this guide walks you through how to open and complete the form from your mobile device.
1. Scan the QR Code or Use the Link
Option A: Scan the QR Code
Open your smartphone’s camera app.
Point it at the QR code until a notification link appears.
Tap the link to open it in your default web browser.
Option B: Open the Link in Your Browser
Tap the provided link or copy and paste it into a mobile web browser (e.g., Safari or Chrome).
2. Accessing the Form
What happens next depends on your BridgeAthletic status:
If You Are Already an Active Bridge User:
Mobile Access:
With the BridgeAthletic app installed and logged in, your browser will automatically redirect you to the form inside the app. No need to enter your email again.
Accessing Via Web Link on a Computer:
You will be prompted to enter in your email to access an invitation link.
Check your email inbox for an invitation.
Click View Form to access the form on your web browser.
If You’re Not Logged In or New to Bridge:
You’ll be prompted to enter your email address in the browser. You are not required to login or register on Bridge in order to access the form.
3. Complete and Submit the Form
Once the form opens (either in-app or via mobile or computer web browser), fill out all required fields.
Tap Submit when finished.

Didn’t get redirected on mobile? Make sure the BridgeAthletic app is installed and you’re logged in.
Form not loading in the app? Try closing and reopening the app. If the issue persists, access the link again or refresh the page.
