Types of Forms on Bridge

The Forms Module on Bridge enables coaches to utilize three distinct form types to collect data, insights, and feedback directly from athletes through the platform.

This article outlines the form types, their purpose, and how athletes and coaches interact with each one.


1. Daily Logs

Assigned as part of an athlete’s daily routine, Daily Logs appear under the Today tab on both mobile and web platforms.

Sub-types include:

  • Performance Log (default)

  • Fitness & Nutrition Log (template option)

  • Custom Daily Log

Coaches may choose to mark a Daily Log as mandatory, requiring athletes to complete it before viewing that day’s workout.


2. Post‑Workout Forms

These forms are triggered automatically at the end of each workout.

Options include:

  • Default Post‑Workout Form: typically consists of 2 questions — RPE (Rate of Perceived Exertion) and workout duration

  • Custom Post‑Workout Form: tailor your own questions to gather specific feedback at the end of each workout.

Athletes are prompted to complete the form before marking the workout as complete.


3. General Forms

General Forms are one-time or occasional surveys sent outside of workout routines. They can be used for compliance checks, screening, onboarding questionnaires, or broader feedback.

  • These forms appear on the Today tab if active, or within a “Forms” section on the platform’s dashboard.

  • Coaches can choose to send a push notification to alert athletes.

  • They remain accessible until submitted.


  • Daily Logs appear automatically under the Today tab and must be completed if flagged as required.

  • After completing a workout, the Post‑Workout Form pops up before the athlete can finish marking it complete.

  • General Forms can be accessed via push notification or through the Forms section on the mobile Forms dashboard.
