Assessment modules hold all form responses and can be found on both the team page and athlete profile page. If you're needing to export form results for just one athlete it is best to do this from their profile page.

First, navigate to the athlete's page and scroll down to the bottom. At the top of the assessment module, click "Export CSV". You will then select the form and time frame. A secured link will be sent to your email to download the CSV directly to your computer.

Form results can also be exported from the team page if you're interested in a mass export for more than one athlete.
