❗These forms currently cannot be customized but we encourage you to send feedback and requests to our customer support team so we can prioritize future form creation.
To send forms through text:
Go to Chat
Search the patient you want to send the form to
Click on the 📄 icon at the bottom of your message to bring up a dropdown list of all your form templates
Select the form template you want, and it will insert the URL to direct your patient to the form to be filled out
To send forms through other communication methods:
Go to Account Settings > Forms and click on the form Link located on the right
Upon clicking the link, you'll see the label change from Copy to Clipboard to Copied!
Now you can paste the copied link into your desired communication channel. You can share this link with your patients via:
Email
A button on your website
Browser using a tablet - to be filled by your patient on arrival at the practice
Any other digital communications channels you can think of
💡When the patient clicks on the link, they will be redirected to your digital form page.
