Log into your Caresmartz360 portal as an Admin or Agency user with permissions to create/update assessments.
Navigate to the Client Profile for which you want to create/update the assessment.
On the left side of the profile, you will see a list of sections for the profile. Click on Assessment.
To add or update an assessment, click on the Green Add button on the top right of the page.
If this is the first Assessment, there won't be any previous assessments in the list. If this is an update, the most recent completed assessment can be copied into the new assessments.
Please capture the Observation Date/Time and mark whether this is a late entry (done earlier and only entered into the system now.)
If you have multiple versions of the assessment built, choose the one you want to use. Otherwise, please select "Default Assessment V-3".
The Facesheet Data will pull from the Main profile information. You can make additions here as well. Make sure you select the Observation Type, Care Plan Update or Reassessment. This will prompt you to select the sections from the most recent completed assessment to be copied into this new version, if desired.
Click "Next" or Navigate to another tab by clicking on it directly.
Needs & Requirements allow you to define the client needs with regard to their care. On the Caregiver side, you can capture Skills and Experience. This will help you with matching clients and caregivers later.
The Attribute Categories and Codes can be configured in the Admin settings by an Admin user in your organization. Admin Settings>Client>Attribute Categories and Attribute Codes.
Under the Tasks Tab, all sections of tasks are visible by default. You can deselect/select the Personal Care/Companion Care/Transportation tasks by clicking on them at the top of the page.
Designate the frequency with which a task should be completed by the caregiver by selecting "Daily", individual days, or "On Request". When making those selections you will be prompted to designate when during the day the task should be completed. You can configure these times in Admin Settings>>Client>>Task Time Preferences.
To the right of the tasks, you can designate that a task only take place during a specific date range, set the Dependency Level "D" or add task specific notes. You can also add client level custom tasks by clicking on the Greed Add Task button.
On the Medications tab, you can add new Medications and reminder schedule by clicking on the Green Add button.
The Medications field is connected to a searchable database. Select the type (Capsule, drop, inhaler, injection, ointment, others, powder, syrup, tablet), then complete the rest of the required fields and Save.
The final tab, Additional Considerations, allows the user to capture other pieces of information such as dietary needs, fall history, transfer/mobility aids, moods & behaviours, allergies, etc. These can be configured in Admin Settings>>Client>>Drop Down Maintenance - the corresponding section.
Once you are done, click on Summary to view the assessment.
If you need to make more edits, click Resume. If you want to come back and make changes later, click Finish Later. When you are done, click Complete. This will lock the assessment and drive its contents, including tasks, into the future visits for this client.
