Form CMS-485 provides a convenient way to submit a signed and dated POC (Plan of Care).
Users are able to download the CMS485 form from the Client’s assessment tab.
They will have the option to download the CMS 485 form from the completed assessment V3 form only.
Once the user clicks on the link “CMS-485”, a slider will open where the users will see some prepopulated details & others will be filled out by the user themselves.
Once they save the form after filling in the information, it will be available in the state mandated format for the user to review.
Clicking on the Download arrow in the bottom left corner will download the completed form.
