CareSmartz offers 4 basic questions as part of the COVID Pre-Screening. You may want to add more.
Navigate to your Office Settings
Select the 3 stacked dots on the far right of the tabs
Select Pre-Screening
Add any new questions you want the caregiver to answer
Make sure that you word your questions so that "Yes" is the adverse response.
