Each patient profile must be created separately using different email addresses. Here's how:
Complete your first profile using one email address
Create the second profile by returning to the sign-up page
Important: If the app remembers your previous login and prompts you to sign in, select "New to Citizen Health? Sign up" instead
Use a different email address for each additional patient profile
Note: Currently, each patient requires their own unique email address and separate account. Family account features are planned for future development.
