How to Upload Medical Records

Select the medical record/folder icon
Tap the folder icon in the bottom-right corner of the screen.

Add a New Record
Tap the “+” (plus) icon in the top-right corner.

Select Upload Record
From the menu options, tap “Upload Record.”

Choose Your File
Tap “Select a file” and choose the document you want to upload from your device.

Log into your account at https://patient.citizenhealth.com/login

Navigate to the upload section: On the "Dashboard" tab, scroll down to "Upload your own records"

Add your files by either:

• Clicking "Select files" to browse your device

• Dragging and dropping files directly into the upload area
