How to Upload Your Medical Records in the Web App
Log into your account at https://patient.citizenhealth.com/login
Navigate to the upload section: On the "Dashboard" tab, scroll down to "Upload your own records" or select the "Records" tab and click "Upload" in the top right corner.
Add your files by either:
• Clicking "Select files" to browse your device
• Dragging and dropping files directly into the upload area
How to Upload Your Medical Records in the Mobile App
Select the medical record/folder icon
Tap the folder icon in the bottom-right corner of the screen.Add a New Record
Tap the “+” (plus) icon in the top-right corner.Select Upload Record
From the menu options, tap “Upload Record.”Choose Your File
Tap “Select a file” and choose the document you want to upload from your device.
