How to update medical record requests

How to Update or Re-Request Records from a Previously Requested Provider

  1. Log in to your Citizen Health profile: https://patient.citizenhealth.com/login

  2. Once logged in, you can access your record requests one of two ways:

Option A — From your Dashboard:

  • View your Dashboard (the initial screen after login)

  • Scroll down to see your medical record requests

  • Find the provider you want to update

  • Click "Request Again" on the original record request

Option B — From the Records tab:

  • Select the "Records" tab

  • Select the "Requests" tab

  • Find the provider you want to update

  • Click "Request Again" on the original record request
