In April 2023, we released an update to Citizen®'s user interface. Our goal with this update was to improve usability for our patients, and to upgrade our design and technology to support a growing user base. As part of this update, some of the features you use may have changed in appearance or moved to different parts of the application. The information below highlights some of the key changes and provides tips for accessing the key functions of Citizen Health®.

What’s New?

Home Page

When you log in to Citizen Health®, you’ll first see a new Home Page. If you’re a new user, this page will walk you through your entire account set up and record request process.

My Health Page

When you select My Health (in the top right corner on Desktop view or in the bottom bar on Mobile) you will see all of the documents that Citizen Health® has collected or you have uploaded, organized by the document type and date they were uploaded. To view the records, click on the thumbnail. You are able to move through the uploaded document page by clicking on the right or left facing arrows at the top of the document that lists the number of pages in each document (for example: ← 1/12 →). You are also able to upload new records in PDF, JPG, and/or PNG format on this page.

Requests Page

The Requests page helps you keep track of your health record requests. On the top of this page, you will see an icon to place new record requests. You are able to update current record requests here by clicking Request Latest Records on the original record request (the original request must be “completed” to request updated records).

Shares Page

The Shares page allows you to control who you share your data with and view any Citizen Health® profiles that have been shared with you.

Viewing Shares

How can I view another user’s profile or documents?

You can view profiles, summaries, or documents that other users have shared with you by clicking on the Shares icon in the top right corner of the home page, and then selecting Shared with me. You’ll see a list of the items shared with you, and clicking on any of these items will navigate to viewing that user’s information in share mode.

How can I see what I have shared with others?

Click on the Shares link on the top right of the screen on Desktop, or the bottom right on your mobile device.

Viewing and Creating Record Requests

How can I place a new record request?

  1. Click on the Requests link at the top right of the screen on desktop, or the bottom right on your mobile device.

  2. Click on the Request Records icon in the top center of the page on desktop, or bottom center of your mobile device to make new record requests.

How can I see the status of a record request?

  1. Click on the Requests link at the top right of the screen on Desktop, or the bottom right on your mobile device.

  2. This will bring you to a list of providers you have already requested records from.

  3. Below the provider name on each request is the status of the request.

How can I update records from an institution I’ve added previously?

  1. To update records from a previously requested provider click on the Requests link at the top right of the screen on Desktop, or the bottom right on your mobile device.

  2. This will bring you to a list of providers you have already requested records from.

  3. Click the Request Latest Records tab on the provider that you would like updated records from (the original request must be “completed” to request updated records).
