Dynamic Forms allow you to collect additional information from attendees during the ticketing process.
Forms can be used for a wide range of purposes, including:
Registration details
Dietary requirements
Waivers and consent forms
File uploads
Electronic signatures
Custom attendee information
Once created, a form can be reused across multiple events, making it easy to standardise your data collection process.
Where to Find Dynamic Forms
You can find Forms under the Settings section in the left-hand sidebar.
From here, you can:
View previously created forms
Create new forms
Edit existing forms
Reuse forms across multiple events
Creating a New Form
To create a new form:
Go to Settings > Forms
Click "New Form"
Enter the required details:
Form Title
Display Text (shown above the form fields to provide context or instructions)
You can then begin building your form by adding fields.
Adding Form Fields
Dynamic Forms support a wide range of field types.
Available field types include:
Short Text
Long Text
Number
Date
Dropdown
Single Choice
File Upload
Checkbox
Downloadable Link/Document
Electronic Signature
You can:
Add as many fields as required
Mark fields as Required
Drag and drop fields to reorder them at any time
This allows you to completely customise the layout of your form.
Form Summary
On the right-hand side of the form builder, you will see the Form Summary.
This provides an overview of your form, including:
Total fields
Required fields
Signature fields
File upload fields
Estimated completion time
This helps you quickly understand the complexity and length of your form.
Form Rules
Below the Form Summary, you will find Form Rules.
These settings control how attendees interact with your form.
Electronic Signature Limit
Only one Electronic Signature field can be added to each form.
Per-Attendee Response
This setting controls how many forms must be completed for each ticket purchase.
When enabled:
Each individual ticket requires its own form to be completed.
If one transaction includes multiple tickets, every ticket holder must complete a separate form.
Example:
If one transaction includes 5 tickets, 5 forms will need to be completed.
When disabled:
Only one form is required for the entire transaction, regardless of how many tickets are purchased.
Example:
If one transaction includes 5 tickets, only 1 form needs to be completed.
This setting is useful depending on whether you need information from every attendee or only from the person making the booking.QR Code Gating
When enabled:
Attendees can purchase tickets normally.
The event QR code will remain locked until the required form has been completed.
This ensures forms are completed before event entry.
Ticket Holder Only
When enabled:
Only the assigned ticket holder can complete their own form.
Completion Notifications
When enabled:
The workspace owner or administrator will receive an email each time a form is completed.
Previewing Your Form
Before publishing your form, you can preview exactly how it will appear to attendees.
To preview your form:
Click "Preview" at the top of the page
This allows you to review the layout and make any final adjustments before saving.
Saving Your Form
Once your form is complete:
Click "Save Form"
Your form will remain available within your workspace and does not expire.
The same form can be reused across as many events as required.
Applying a Form to a Ticket Tier
Dynamic Forms are applied to individual ticket tiers.
Note: A ticket tier must already exist before a form can be assigned.
You can apply a form in two ways.
When Creating a Ticket Tier
During ticket tier setup:
Select the Dynamic Form you wish to use
After a Ticket Tier Has Been Created
Go to Ticket Management
Edit the required ticket tier
Scroll to Dynamic Form (Optional)
Select the form you want to assign
Save your changes
Once assigned, every attendee purchasing that ticket tier will be prompted to complete the form.
Completing a Form as an Attendee
After purchasing a ticket, attendees will be prompted to complete any assigned Dynamic Forms.
Attendees can access the form from:
The Tickets section of the Attendee Portal by clicking "Fill Out Form"
Their ticket confirmation email, which also contains a "Fill Out Form" button
Once submitted, the form will be linked directly to the attendee's ticket.
Viewing Form Responses
Completed forms can be viewed from Guest Management within your event.
From Guest Management, you can:
See which attendees have completed their forms
View submitted responses
Access information provided by attendees
Under More Actions, you can also:
Export Form Responses
This allows you to download all submitted responses for reporting or external use.
Resetting a Completed Form
Once a Dynamic Form has been submitted, the attendee can no longer edit their responses.
If changes are required, a workspace owner or administrator can reset the form.
To Reset a Form:
Go to Guest Management
Click "Form Complete" for the attendee
Select "Reset Form"
The attendee will then be able to complete the form again.
