Print out the form and complete it. Include a witness signature.
Scan* and upload the file in the cleaner application form
*Please don't take a photograph on your smartphone of this form as it won't be legible.
Can you use an online tool like DocuSign?
Yes you can, however, your witness signature has to be via this method too.
