  1. Certificate of Good Standing: Not Required

  2. Agency: Required 

  3. Agent: Required*

  4. Underwriter: Please note any and all underwriter appointments    

Insurance: 

  1. E&O: Required

  2. Fidelity Bond: Not Required 

  3. Surety Bond: Not Required 

  4. Other: Upload Wire instructions to "Manage My Bank Accounts". 

*Notes & Exceptions: In the case that you are an attorney, fidelity bonds, surety bonds, and agency license will not be required.  Please provide an attorney bar card in PDF form in place of the agent license.
