Turn Edit mode on.

Click the + sign in the section in which you would like to add the activity. Then click Activity or resource.

Select the Choice tile.

Name the activity, type the question in the description box, and click Display description on course page box (optional).

Choose how you want the options displayed.

Decide if students should be able to change their answer or make more than one choice.

Fill in the response choices.

Set other activity criteria as you normally would.

Save your changes.

The activity will display on your course page.

This is how the activity displays for the students if you choose Display horizontally.

This is how the activity displays for the students if you choose Display vertically.

Click the Responses tab to see student choices.
