1Form V3.5 Updates

Several enhancements have been made across the 1Form experience to streamline data collection, improve clarity, and ensure regional compliance. The summary below outlines the updates for version 3.5, organized by section, for easy reference.

Business Section

Business Details (US Only)

  • A new checkbox under the W-9 upload allows users to indicate if they are a disregarded entity filing taxes through a parent company.

Contacts

  • A new question asks for a Business Development contact, including name, title, email, and phone number.

Diversity Certifications

  • Each certification now includes a question on whether it was issued at the state/province or national level.

  • Selecting state/province reveals additional dropdowns for the location and issuing body (issuing body is only available on the US 1Form).

Insurance Policies

  • This section was moved from Finance to Business.

  • All policies (except Workers’ Compensation and Employers’ Liability) now require both a Single Occurrence Limit and Aggregate Limit.

  • A checkbox is provided for Workers’ Compensation and Employers’ Liability to indicate no specified limits, which hides the limit fields.

Construction Licenses/Certifications

  • Added "Building Information Modeling (BIM)" across all 1Forms.

  • Added "New York State Department of Labor (NYSDOL Registration)" to the US 1Form only.

ISO Certifications

  • Certifications must now be entered as separate items (not checkboxes).

  • Each requires a document upload and, where applicable, an expiry date.

Expertise

  • A new consolidated page replaces Contract Size, Project Type, and Trade Scope Expertise sections.

  • Users must provide total contracts in the last 3 years and select common contract value ranges from a dropdown.

  • Users can choose up to three project types and up to three trade scopes to specify.

Legal

  • New questions include legal representative type, current litigation status, and judgments in the last 3 years.

  • A new dropdown captures reasons for recent changes in ownership, control, or management.

  • Legal language across the section was streamlined for clarity.

Employee Details

  • The question on average number of field workers was moved here from the Tradespeople and Unions section.

Employee Turnover

  • For turnover rates of 11% or more, users must select a reason from a dropdown or specify if "Other" is chosen.

Subcontracting

  • The option “We don’t subcontract out work” was removed from the follow-up question, as it’s already included in the primary subcontracting question.

Tradespeople and Unions

  • The field worker count question was moved to the Employee Details section.

Largest Ongoing Projects

  • Renamed from “Largest Current Projects.”

  • A Project Type dropdown was added.

  • A checkbox allows hiding the zip code field if unavailable.

  • Users can now add multiple CSI scopes of work.

Largest Completed Projects

  • Received the same updates as the Largest Ongoing Projects section except users must specify a zip code.

Average Contract Value

  • The label was updated to “Estimated Average Contract Value.”

Diversification by Province/State/Region

  • Page removed entirely.

Finance Section

Most Recent Fiscal Year End Financial Statements

  • Renamed from “Most Recent Year End Financial Statements.”

  • Copy and labels updated to clarify requirements on the page.

Prior Fiscal Year End Financial Statements

  • Renamed from “Prior Year End Financial Statements.”

  • Copy and labels updated to clarify requirements on the page.

Banking

  • Users can now specify multiple lines of credit if they answer Yes to having one.

  • Each credit entry includes institution, currency, expiry/maturity date, total capacity, amount drawn, and a file upload for the operating credit letter.

Performance Bonding

  • New “Security Details” section added with questions on funds control and financial/performance security.

    • Users can select from security types such as Personal Guarantee, LOC, Parent Company Guarantee, or Other with a textbox to specify.

  • Under the bonding limits section, users now specify rate structure (single or different based on amount).

  • Single rate reveals one dollar field; different rates reveal multiple fields to be populated.

  • Users can add extra lines using “+ Add line.” to reflect the structure of bonding letters.

  • If a company has had claims against its bond, an explanation and document upload are required.

  • Bonded projects introduced with a minimum of three required. Each entry must include a project name, contract value, and year completed/estimated.

Health and Safety Section

Health and Safety Program

  • Added 11 new checklist items under “Please check procedures included in the Health and Safety program,” including programs for IIPP, Fire Prevention etc.

  • Moved the questions “How often does your company hold a job site safety meeting?” and “How often do you hold regular safety meetings for field supervisors?” from the EMR section to Health and Safety on the US 1Form.

Workers Compensation Boards

  • Removed the CAD-7/NEER upload requirement for Ontario.

Citations or Fines (US Only)

  • If the user selects “No” to having convictions, fines, or interventions in the past 5 years, a file drop appears requesting a screenshot of their OSHA citation log from the OSHA website.

OSHA 300A

  • Renamed from “OSHA 300.”

  • New question asks if the user is required to submit injury and illness data to OSHA.

    • If No, users choose from predefined reasons or specify “Other.”

    • If Yes, users complete data for 3 years, or check “Cannot provide incident info” for any year.

      • If checked, reasons must be selected (e.g., new company, small business, etc.) and an optional document field appears.

  • Removed fields: First Aid Cases, OSHA Inspections, OSHA Violations, and old OSHA 300 file drop.

  • File drop now titled: “Attach OSHA’s Form 300A” with a redaction disclaimer.

EMR

  • First question updated to “Do you qualify for EMR?”

    • If No, users select a reason and upload confirmation.

    • If Yes, states checkboxes are shown as before.

  • Effective dates have taken place of expiry dates.

  • A checkbox lets users indicate they do not have EMR for a given year, triggering reason options and support file uploads.

ESG

Canadian ESG Section

  • Added Anti-Slavery and Human Trafficking page to the Canadian 1Form, using identical questions from the UK version.

US ESG Section

  • Introduced a full ESG section to the US 1Form, including two subsections from Canada:

    • Environmental: Climate-Related Targets/Goals, Emissions and Fuels, Environmental Product Declarations (EPD).

    • Governance: Third Party Reporting and Other ESG Documents.

  • Added a new Compliance page with structured Yes/No questions covering anti-bribery and corruption policies, employee training, government affiliations, and hospitality or gift practices.

    • For each “Yes” response, users are prompted to provide an explanation and upload supporting documentation.
