Potentially Notifiable Incidents

The Notifiable Incidents feature is an optional configuration in the Incident Module. When it is enabled it will ask the User if the Incident is potentially notifiable immediately after creating the Incident record. The workflow can be customised at this point but usually involves escalating the Incident to the Security Group (i.e. with restricted access). See also Notifiable Incidents - Legal.

A potentially notifiable incident (PNI) usually refers to an event or situation in the workplace that has the potential to cause significant harm or danger but may not have resulted in serious injury or illness at the time it was recognised. These can include near misses, unsafe conditions, or hazardous situations that, if left unaddressed, could result in a serious incident.

In Australia, under the Work Health and Safety Act, a "notifiable incident" involves the death of a person, serious injury or illness, or a dangerous incident that exposes a worker or any other person to a serious risk. A "potentially notifiable incident", in this context, might be something that has the potential to result in one of those outcomes.

This guide shows the workflow for creating a Potentially Notifiable Incident.

1.After entering the Incident details (step 1) and selecting continue, the following pop-up message will appear:

2. You will have to select which one is applicable:

2.1 If it is not potentially notifiable → it will go straight to the normal workflow to add the details of the incident.

2.2 If it is potentially notifiable → it means that the Security Group configured for Notifiable Incident workflow will be notified, so it will take a different workflow and it will provide instructions on what to do next (an exception here might be if you are part of the Security Group).

Note that the instructions displayed to the user at this point will be specific to each organisation and process. Typically the user will be prompted to call someone from the security group and the text would explain that they may not be able to continue editing the Incident after continuing.

3. A modal is displayed to the user with instructions and the option to provide their best contact phone number (i.e. one that can be contacted on immediately). Once this is done the user is able to select Okay to continue.

4. At this point the “Potentially notifiable” Incident, will become restricted, which means that only users allocated to the potentially notifiable security group will have access to view and make changes to the incident.
