What is the Merchant Surcharge Disclosure Form?

The Merchant Surcharge Disclosure Form is a document that Mastercard requires US merchants to submit prior to applying a surcharge to customers who use Mastercard credit cards for payment. Specific Mastercard rules (US) regarding surcharging can be found here.

Why Do You Need the Form?

The form is intended for your business to comply to their regulations to maintain a well-regulated, consistent, and transparent payment process.

Merchants are responsible for ensuring compliance with all applicable surcharging rules established by accepted card networks and local jurisdictions.

Completing the Merchant Surcharge Disclosure Form

To register your business with Mastercard, please use the following link: Merchant Surcharge Disclosure Form.

Follow the steps below to complete the registration:

  1. First Name, Last Name, Email Address, Job Title: Enter the account owner’s information.

  2. Organization: Provide your legal business name.

  3. Address, City, State, Zip Code: Enter the specific address of your business. If you have multiple locations, please note that each location will require its own form.

  4. Merchant Name (Doing Business As - DBA): Enter the DBA name listed in your Stripe account dashboard.

  5. Acquirer/Processor Name: Enter "Stripe."

  6. Type of Surcharge: Select "Brand."

  7. Payment Channel:

    • Select "Retail (face-to-face)" for in-person transactions.

    • If you also require deposits through DaySmart’s online booking tool, select "eCommerce."

  8. Date Surcharge Will Begin: Enter the date of when you plan on surcharging.

  9. Number of Locations Surcharging: Enter the number of locations or DaySmart accounts that will be utilizing surcharging. Reminder that each location requires its own form.

  10. Check the agreement at the bottom of the form.

  11. After completing all fields, select "Submit."
