Get valuable feedback from patients after they come in for a consult

Get feedback from new patients with my New Patient Tracker!


After they keep their appointment, I'll send a Thank You message (Settings >> Ava's Follow Up >> NP Survey) with a link to this survey.

If they don't respond they'll get up to 2 additional messages requesting that they complete the survey (Ava's Follow Up >> NP Survey)

Survey Questions

There are 4 sections to the survey.

View Survey Results

Answers are anonymous. When 5+ patients have complete it, you'll be able to see the average response to each question in a "Report Card".

You can see who has submitted a survey however, indicated with a blue checkmark in the "Survey" column (on the NPT page).

Click "View Survey Data":

Survey Report Card - Q&A Breakdown:

You can see your average score at the top, in each of the sections of the survey.

From there you'll be able to see the average responses in a really detailed breakdown of patients answers, including any written feedback:
