Instruction on using your real name when creating and sending a DMCA Takedown notice?

As the Digital Millennium Copyright Act takedown notice is a legal document you are required to use your legal name (see Professionally Managed takedown option below for exception).  

Although DMCA.com is unable to provide legal or copyright advice, it is our understanding that attempts at deception when creating a takedown notice may be met with harsh legal penalties. 

The Professionally Managed takedown service has had success processing takedown notices without using our client's real or registered name.  When completing your takedown notice here, make certain to make the name removal request within the description of notice.  Our takedown professionals will contact you to confirm the process.  You will need to use your real registered name when creating the takedown notice with DMCA.com. 

Request for ID from websites and hosting companies
There are some instances where site hosts request proof of identity. While this is not a common practice is does occasionally occur.  We recommend under privacy and safety concerns that you do not provide any website or hosting company proof of identification.
