How to Manage Applicants Using Application Forms

A step-by-step guide on sending application forms, creating applicants, onboarding drivers, and managing carrier associations in DQID.

Step-by-Step Process

Step 1: Send Application Form (Manage Applicant)

  1. Navigate to the Manage Applicant tab.

  2. Click on Mail Form.

  3. A modal will open with the following options:

    • Email the application form link to the driver

    • Copy link to share manually

Carriers can send this form to any driver.


Step 2: Add Waivers (If Required)

  1. Click on the Carrier Waivers button.

  2. A modal will open listing all available checklist items.

  3. Select one or more checklist items to include as waivers in the applicant form.


Step 3: Access Application Form

  1. The applicant receives an email containing the link to the application form.

  2. The driver fills out and completes the Application Form.

  3. On clicking "Submit", instead of final submission, the system transitions the driver to the next step: Waivers.


Step 4: Waiver Step (Sequential Document Signing)

  • The driver is presented with a series of waiver documents (pulled from the checklist) step by step.

  • For each document:

    • The driver reads the document

    • Provides a digital signature to acknowledge and consent

    • Clicks "Next" to proceed to the next waiver


Step 5: Final Submission

  • Once all waiver documents are signed, the system submits the entire packet:

    • ✅ Application Form

    • ✅ All signed waiver documents


Step 6: System Actions

  • The signed documents are automatically added to the driver’s checklist as a pending file

  • The packet is marked as fully submitted for review or further processing by the carrier/admin


Step 7: Guest Form Access (New Driver)

  • If the driver does not exist in DQID, the emailed link opens the form in Guest Mode.

  • The driver can:

    • Fill out the form

    • Complete the waivers

    • Submit the full packet without logging into DQID


Step 8: Applicant Creation

  • Once the driver submits the full packet:

    • A new Applicant is automatically created in DQID

    • The applicant appears under the Applicant tab for the relevant carrier


Step 9: Carrier Review & Approval

  • The carrier reviews the submitted packet

  • Upon approval:

    • The applicant is converted into a Driver

    • The onboarding process is completed


Step 10: Carrier Association

  • After approval, the driver is automatically associated with the carrier for whom the form was submitted


Step 11: Driver Profile Creation

  • A Driver Profile is created in DQID

  • Profile fields are pre-filled based on the submitted application form


Step 12: PDF Generation

  • A PDF is generated for the submitted application form and signed waivers

  • The PDF is attached to the carrier checklist: Driver Application / Questionnaire


Step 13: DQ File Verification

  • An entry is automatically added under DQ File Verification

  • The packet follows the standard verification workflow, similar to other DQ documents


Special Scenarios

Scenario 1: Existing Driver Associated with a Carrier

  • If a driver already exists in DQID and is associated with a carrier, submits the application for another carrier:

    • The application appears under the Applicant tab for the new carrier

    • When approved, the driver receives a notification asking:

      Your application for this carrier has been approved. Would you like to leave your existing carrier?

    • The system proceeds based on the driver’s decision

Scenario 2: Applicant Applies to Multiple Carriers

  • If a driver submits the application form for multiple carriers:

    • The entry appears for all selected carriers

    • Once any carrier approves, the status for other carriers is automatically updated to Withdrawn

    Frequently Asked Questions (FAQs)

    1. Who can send an application form?

    Application forms can be sent by Carrier Admins or Admins from the Manage Applicant tab.

    2. Can an applicant submit the form without having a DQID account?

    Yes. If the driver does not exist in DQID, the form opens in Guest Mode, allowing the applicant to complete and submit the application and waivers without logging in.

    3. What happens after the applicant submits the application form?

    After submitting the application form, the applicant is guided to the Waiver step, where all required waiver documents must be reviewed and digitally signed before final submission.

    4. Are waivers mandatory for all application forms?

    No. Waivers are optional and only appear if the carrier selects checklist items as waivers while configuring the application form.

    5. How are waiver documents presented to the applicant?

    Waivers are shown one at a time in a sequential flow. The applicant must read, sign, and click Next to proceed to the following waiver.

    6. What happens after all waivers are signed?

    Once all waiver documents are signed:

    • The entire packet (application form + waivers) is submitted

    • Signed documents are added to the driver’s checklist as pending files

    • The submission becomes available for carrier review

    7. When is an applicant created in DQID?

    An applicant is automatically created after the full packet is submitted, including the application form and all required waivers.

    8. What happens when a carrier approves an applicant?

    When approved:

    • The applicant is converted into a Driver

    • A Driver Profile is created with prefilled information

    • The driver is automatically associated with the approving carrier

    9. Is a PDF generated for the submitted application?

    Yes. A PDF is generated and attached to the carrier checklist under Driver Application / Questionnaire.

    10. Will this submission appear in DQ File Verification?

    Yes. An entry is automatically added to DQ File Verification, following the same verification process as other DQ documents.

    11. What if the applicant already exists as a driver for another carrier?

    If the driver is already associated with a carrier:

    • The application appears under the new carrier’s Applicant tab

    • Upon approval, the driver receives a confirmation request asking whether they want to leave their existing carrier

    • The system proceeds based on the driver’s decision

    12. What happens if an applicant applies to multiple carriers?

    The application appears for all carriers. Once any one carrier approves the application, the status for the remaining carriers is automatically marked as Withdrawn.
