Overview
Drivers can apply for DQID by completing an Application Form provided by a carrier. The form can be accessed in multiple ways, allowing drivers to apply easily whether they receive a direct invite or access it from a carrier’s website.
How to Access the Application Form
Drivers can access the application form in three ways:
Invite via Email
The carrier sends the application link to the driver’s email.
The driver clicks the link to start the application.
2. Direct Link Shared by Carrier
The carrier shares the application link directly through any app (for example, messaging apps).
The driver opens the link to access the form.
3. Carrier Website
If available, the driver can access the application form directly from the carrier’s website.
Step-by-Step Application Process
Step 1: Open the Application Form
The driver opens the application link.
The system prompts the driver to enter their email address.
Once the email is entered, the full application form is displayed.
Step 2: Complete the Application Form
The driver fills out all required fields in the Application Form.
After completing the form, the driver clicks “Submit”.
Note: Clicking Submit does not finalize the application. The driver is taken to the waiver step.
Step 3: Waiver Step (Sequential Document Signing)
The driver is presented with a series of waiver documents pulled from the carrier’s checklist.
Waivers are shown one at a time.
For each waiver, the driver:
Reads the document
Provides a digital signature to acknowledge and consent
Clicks “Next” to proceed to the next waiver
Step 4: Final Submission
After all waiver documents are signed, the system submits the entire application packet, which includes:

✅ All signed waiver documents
What Happens After Submission
The application is sent to the carrier for review.
The driver will receive an email update based on the carrier’s review and approval process.

1. Do I need a DQID account to apply?
No. You can complete the application and sign all required waivers without creating or logging into a DQID account.
2. How will I receive the application form?
You may receive the application link:
Via email from the carrier
As a direct link shared through messaging apps
From the carrier’s website, if available
3. Why am I asked to enter my email before the form opens?
Your email is used to:
Identify your application
Link your submission to the correct carrier
Send updates related to your application
5. Can I skip or partially sign waivers?
No. All listed waiver documents must be read and digitally signed to complete the application.
6. What happens after I submit the full application?
Your application is sent to the carrier for review. You may receive further communication depending on the carrier’s approval process.
7. Can I edit my application after submission?
Yes, you can edit the form by accessing the link again while it is still under review. Once the carrier reviews the application (approved or rejected), changes can no longer be made
